I lost the token and forgot the password. Now what, do I start all over?

Take it easy, breathe. If you lost the A3 token or forgot the password, you usually can't recover the old certificate. It becomes unusable, and the typical path is to issue a new one. Annoying, but it solves the problem. That's why TF Con usually recommends the A1 model, which is stored in a secure file, with no token to go missing in a drawer.
